Resident Status:   Must be a resident of the state of Kentucky.  The state of Kentucky does not issue non-resident CCDW Licenses.

Where To Obtain Application:  Anyone wanting to receive a CCDW Licens, must obtain an application from the Sheffif's Office in their resident county.

Application Form:  This form is completed under oath and includes:

• Name of applicant,
• Address,
• Date of Birth,
• Gender of applicant,
• Social Security number,
• Photo of applicant (supplied by applicant and can be no smaller than 3 1/2 " X 4" or larger than 4" X 5".
• Photocopy, copy, or affidavit of sucessful completion of the training requirements approved by the Kentucky Department of Criminal Justice Training.
